Maple Grove Family Health has been part of this neighbourhood for over 30 years. What began as a single-physician practice in a converted Victorian home has grown into a three-physician team, while keeping the same unhurried, personal approach.

We provide comprehensive care for individuals and families at every stage of life, from newborns to seniors. Our team works together so your care is coordinated, consistent, and always familiar.

Comprehensive Care
Under One Roof.

From routine checkups to chronic disease management, our team provides a full range of primary care services. Most services are covered by OHIP.

Preventive Care & Checkups

Annual physicals, immunizations, health screening, and chronic disease monitoring for all ages.

Mental Health & Seniors Care

Counselling referrals, medication management, cognitive screening, and fall prevention for older adults.

Pediatric & Women’s Health

Well-baby visits, childhood immunizations, Pap smears, prenatal coordination, and reproductive health.

Prescriptions & Referrals

New prescriptions, renewals, and specialist referrals. Please allow 2 to 3 business days for renewals.

Virtual Appointments

Secure video visits for eligible follow-ups, prescription renewals, and minor concerns. Available to existing patients.

Non-OHIP Services & Fees

Some services such as medical notes, forms, and insurance documentation carry a fee. Ask our front desk for the current fee schedule.

Dr. Chen founded Maple Grove Family Health in 1994 and has been the cornerstone of the practice for over 30 years. She completed her medical degree at the University of Toronto and has a particular interest in preventive care and supporting patients through major life transitions.

Dr. Webb joined the practice in 2010 and focuses on chronic disease management and sports medicine. He completed his medical degree at Queen's University and holds a certificate in Sport and Exercise Medicine from the Canadian Academy of Sport and Exercise Medicine.
