Fill out the new patient registration form online or pick one up at the front desk. This helps us prepare for your first visit.
Our team reviews your form within 2 business days. We may contact you with a few questions before confirming.
Once confirmed, we will contact you to book your first appointment. First visits are typically 30 minutes.
What to Bring
Good to Know
Most services are covered under OHIP. Some services including medical notes, forms, and certain procedures may have a fee. A fee schedule is available at the front desk.
We can request your records from your previous physician. Please bring their name and contact information to your first visit.
Many follow-up visits and minor concerns can be handled by secure video. Ask our team when booking if a virtual appointment is right for your visit.
New patients only need to complete the New Patient Registration to get started. Once we have reviewed your registration and confirmed your first appointment with Dr. Nair, we will reach out to request any supplemental forms that may apply to your care.
