Please allow 2 to 3 business days for prescription renewal requests. Renewals require a recent visit within the past 12 months. If you have not been seen recently, you may need to book an appointment before your prescription can be renewed.
How to Request a Renewal
Call our office during business hours. Have your pharmacy name and phone number ready. We will send the renewal directly to your pharmacy.
Controlled Substances
Renewals for controlled substances require an in-person or virtual appointment. These cannot be processed by phone request alone.
Fax Requests from Pharmacies
Your pharmacy may fax a renewal request directly to us at (905) 555-0143. Please note that fax requests still require a recent visit on file.
Running Low?
Request your renewal at least one week before you run out. Last-minute requests cannot always be accommodated the same day.
How Referrals Work
Referrals to specialists require an appointment with your family physician. We cannot process referral requests by phone without a recent visit.
Your doctor will send a referral letter to the specialist on your behalf. Wait times vary depending on the specialty and urgency. We will notify you once the specialist contacts you to book.
Checking Referral Status
If you have not heard from a specialist within 4 to 6 weeks, contact our office and we can follow up on your behalf.
No news is not always good news. If you have not heard about your results within 2 weeks, please call our office. We will always contact you if results require follow-up, but it is your responsibility to confirm receipt.
How We Share Results
Normal results are typically communicated by phone. Abnormal results will always be discussed with you directly, either by phone or at a follow-up appointment.
Cancellations
Please give us at least 24 hours notice if you need to cancel or reschedule. Repeated missed appointments without notice may result in a fee.
Late Arrivals
If you arrive more than 10 minutes late for your appointment, we may need to reschedule. Please call ahead if you are running late.
One Issue Per Visit
To ensure each patient receives adequate time, we ask that you focus on one primary concern per appointment. If you have multiple issues, please let us know when booking so we can schedule appropriately.
Privacy
Your health information is kept strictly confidential in accordance with Ontario’s Personal Health Information Protection Act (PHIPA). We do not share your information without your consent.

Our office is closed evenings and weekends. If you need care when we are not available, the following options are available to you.

Non-OHIP Services
Fee Schedule
Some services are not covered by OHIP and carry a fee. This includes medical notes, forms, insurance documentation, and certain procedures. A current fee schedule is available at the front desk — please ask our team for details.

How do I book an appointment?
Call our office at (905) 555-0142 during business hours. We offer in-person and virtual appointments. Same-day appointments are available when possible.
What if I need a sick note?
Medical notes and forms require an appointment and carry a fee. Please allow 5 to 7 business days for completion. Ask our front desk for the current fee schedule.
How do I get a copy of my medical records?
Complete a records release form at the front desk. A fee may apply. Please allow up to 30 days for processing.
Do you offer virtual appointments?
Yes. Many follow-up visits and minor concerns can be handled by secure video. Ask our team when booking if a virtual appointment is appropriate for your concern.
